Issues of abuse and neglect can be reported to the Paulding County Board Of Developmental Disabilities by staff, individuals with disabilities, families, providers or the general public. 

Who Is To Report

MUIs may be reported to PCBDD by staff, individuals with disabilities, families, providers or the general public.
All providers who are contracted, certified, or licensed to serve persons with developmental disabilities are required to report MUIs, as well as all Paulding DD staff. MUIs should be reported immediately, but no later than four hours after becoming aware of the incident.

How To Report

During normal business hours, call the PCBDD at 419-399-4800 and ask to speak to an SSA.

❖ Your call will go to Family Service of Northwest Ohio (Answering Service)
❖ Explain your emergency and give contact information
❖ The answering service will contact the SSA on call
❖ An SSA will return your phone call
